Proof of Age document is required for all members and tournament players. The following documents will be accepted for the Proof of Age documentation requirement:
- A clean and readable copy of the player’s certified county, state or foreign country birth certificate. A hospital “Keepsake” certificate is not an accepted form of documentation and will not be approved.
- A clean and readable copy of the player’s valid passport. Expired passports will not be accepted.
- A clean and readable (*Notarized) copy of the players state or foreign country drivers license.
*The only document that needs to notarized when submitted is a copy of a player’s drivers license.
Do not send original documents, they will not be returned.
Documents may be emailed, faxed or mailed and must be submitted for review and approved before tournament participation is approved.
